We are only able to take the first 20 participants under each time frame.  You MUST list ALL additional attendees in this form such as children, parents, etc. so that we can account for them as well. If you are unable to attend, please email m.shore@havanail.gov and let us know ASAP, so that we can let the other applicants know they are able to attend if they still wish.
